If you have workplace benefits or Freedom to Choose™ health and dental insurance and want to add a dependent to your benefits, you must complete and submit an employee change form.
- Find it in Forms under the Administrative tab.
- If it’s not there, request one from your plan administrator (usually the human resources person at your workplace).
